If you purchase from a bookseller other than Amazon or Barnes&Noble, please input the name of the bookseller in the "Order Number" field of the form, along with your Order Number (if you were provided with one).
	How do I find my Order Number? If you ordered the book online, you can usually find your Order Number on the Order Confirmation page after you have completed your purchase. You should also be able to find it in the Order Confirmation e-mail sent to you by the bookseller after you placed your order.

	How do I complete the form? Please enter the requested information into all fields and click on the "Submit" button. FIRST NAME: Enter your First Name. LAST NAME: Enter your Last Name. EMAIL: Enter your E-mail Address. RETYPE EMAIL: Retype your E-mail Address for verification purposes. PHONE NUMBER: Enter your Phone Number, including area code and international dialing code (if appropriate). COUNTRY: Enter the Country in which you live. ORDER NUMBER: Enter your Order Number. If you purchased from a local bookseller, please type in the name of the bookseller. QUANTITY ORDERED: Enter the number of books you ordered. If you purchased more than one book, please select "Multiple". If you did not purchase a book and would just like to enter the contest, please select "Zero". SELECT THE PRIZES YOU ARE INTERESTED IN: Select the specific prizes you would like to be entered to win. 	Do I have to complete the form? You need to fill in the form completely in order to: Enter the contest to win one of the prizes. 	Why do you request my phone number? We request that you submit your phone number so that Customer Service can contact you if you win a prize. If you do not wish to enter the contest then you do not need to enter your Phone Number and you can select "None" in the Prize options.

	How do I access the bonus gifts? In order to access the bonus gifts, you must first purchase the book and then complete the form on the promotion page. Once you have completed this form, you will be directed to the Bonus Gifts page where you can access ALL of the gifts. Each gift may have different access options (i.e. direct download of the gift, gift opens directly in your browser window, link to the sponsor's gift page for access, etc.).
